**Experienced Part-Time Housing Support Specialist – Remote Case Management and Community Outreach**

Job Summary

We're seeking an experienced and compassionate Housing Support Specialist to join our team on a part-time basis. As a Housing Support Specialist, you'll work closely with Loretto Hospital staff to identify, engage, and assess individuals exiting the hospital without a housing destination. You'll provide intensive case management services, connect individuals to eligible resources, and facilitate their transition to stable housing. This is a remote position that offers an immediate start for the right candidate.

Key Responsibilities

  • Provide intensive mental, physical, and social health case management services at Loretto Hospital and Renaissance Engagement Center.
  • Work with hospital staff to identify individuals in need of housing and build rapport while still hospitalized.
  • Assess eligibility for Coordinated Entry System and our Housing Navigation program.
  • Identify client needs, including barriers to receiving appropriate services and assist with effective problem-solving to address issues.
  • Provide treatment planning, coordination, and implementation of services needed.
  • Provide information, linkage, referral, and advocacy for clients with agencies in the community, including crisis intervention services when needed.
  • Maintain the cleanliness and atmosphere of the Renaissance Engagement Center.
  • Work with clients to increase their skills and income, improve their self-determination, and achieve residential stability.
  • Maintain the confidentiality of client information in any written, oral, or electronic form, including health information, HIV/AIDS status, and disability information.
  • Report all incidents of abuse and/or neglect as required by state law.
  • Maintain accurate and up-to-date records using HMIS, SharePoint, our EHR, and paper files on all clients receiving services.
  • Coordinate the provision of appropriate services from other providers in the community.

To succeed in this role, you'll need to possess the following core competencies:

  • Ability to recognize and assess the signs and symptoms of mental illness, physical illness, development limitations, and substance-induced behavior and know what proper treatments are for these issues.
  • Ability to communicate relevant information in writing in a clear, concise, and appropriate manner with other staff, supervisors, and program participants.
  • Ability to communicate relevant information verbally in a clear, concise, and respectful manner with other staff, supervisors, and program participants.
  • Ability to work in a team environment, share information, and coordinate services with other staff and external organizations.
  • Ability to complete work independently without prompting and maintain a schedule and follow through with tasks without prompting.
  • Ability to ask for input and assistance from your supervisor and accept supervision when needed.
  • Ability to complete required paperwork before deadlines and manage time wisely to follow through with assigned tasks as soon as possible.

Requirements

To be considered for this role, you'll need to meet the following requirements:

  • High School diploma or equivalent plus related experience, or a Bachelor's degree in a related field.
  • Experience in mental health services preferred.
  • Experience with Harm Reduction techniques, Housing First, and substance use is desired.
  • Computer literacy is a must!
  • Candidates with an automobile and current insurance preferred.

  • Experience working with vulnerable populations, including those experiencing homelessness or mental illness.
  • Knowledge of community resources and ability to connect clients with relevant services.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and staff.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and coordination.
  • Experience with case management software and electronic health records (EHRs).
