About Our Clinical Stars Team
The Clinical Stars Team is a dedicated group of healthcare professionals, including Clinical Pharmacists and Pharmacy Technicians, who work together to deliver high-quality patient care through various initiatives. Our services include strategic Medication Adherence outreach programs, Care of Older Adults (COA) medication reviews and health assessments, and clinical consultations for providers. As a Clinical Pharmacist on our team, you'll have the opportunity to work closely with patients, healthcare providers, and other clinicians to optimize medication therapy, improve patient outcomes, and drive business results.
Key Responsibilities
- Conduct telephonic medication consults with patients, meeting or exceeding internal and external quality metrics for various programs, including United HealthCare at Home (UAH), COA, and SUPD/SPC.
- Identify and address patient-specific medication adherence barriers, collaborating with healthcare providers and pharmacies to overcome obstacles and improve STARs ratings.
- Implement a health-literate, patient-centric approach in all clinical consults, ensuring effective communication and education.
- Participate in team meetings and coaching sessions to maintain and improve clinical interaction skills and overall performance.
- Support Advanced Practice Clinicians (APCs) through telephonic clinical consultations, providing expert guidance and recommendations.
- Meet or exceed established productivity, quality, and health literacy metrics on a weekly and monthly basis.
- Engage in various campaigns and initiatives throughout the year, as assigned by the Clinical Team Manager (CTM).
Essential Qualifications
To succeed in this role, you'll need:
- A current, unrestricted, and active pharmacist license in the state where you're practicing.
- A Doctor of Pharmacy (PharmD) degree from an Accreditation Council for Pharmacy Education (ACPE)-accredited program.
- Passing the North American Pharmacist Licensure Exam (NAPLEX).
- 2+ years of experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint.
- 1+ year of Clinical Pharmacy Practice experience.
Preferred Qualifications
While not required, the following qualifications can enhance your application:
- An MBA or MPH degree.
- Post-graduate training, such as a Pharmacy Residency or Fellowship.
- Board certification by the Board of Pharmacy Specialties (BPS).
- Certified Geriatric Pharmacist credential.
- 1+ year of Call Center experience.
- Experience with Medication Therapy Management (MTM) and Comprehensive Medication Reviews.
- Familiarity with Medicare STARs and HEDIS Measures.
- Proficiency in Microsoft Office applications.
- Bilingual skills in languages such as Spanish, Mandarin, Cantonese, Korean, Russian, or Vietnamese.
