Job Summary
The HTS of the Home Integration Supported Living Homes are responsible for the day to day operations of the Supported Living Homes under the general direction of the House Manager, Program Coordinator, Administrative Director, Associate Director, and Director.
Performance Requirements
It Preforming personal care task, including assistance with basic personal hygiene and grooming, feeding, ambulation, medical monitoring, and health related task.
• Ensuring clients safety and maintaining a safe environment
• Assisting with toileting, including urinals, and commodes
• Encourage self-help activities
• Documenting services provided
• Performing home management functions such as light housekeeping, laundry, bed making, and cleaning

• Planning meals, shopping for groceries, preparing and serving food/meals, feeding and clean-up
• Accompanying clients to scheduled and unscheduled appointmehts
• Transporting clients and/or running errands for clients
• Following all programming
• Filling out all paperwork that is needed for the clients in the home
• The health and welfare of the clients
• Report all maintenance issues to the House Manager
14.Follow all policies, rules, and regulations
15. Any other duties required by the needs of the clients, Team, House Manager, Case Manager, Program Coordinator, Administrative Director, Associate Director, and Director.
