I need a skilled freelancer to design a professional vendor form similar to the attached sample from. Remove Canadian health authority fields (FHA, IHA, etc.) and non-U.S. tax sections. Format in a clean, editable PDF template (e.g., using LaTeX or a design tool like Adobe Acrobat). Include a section for required documentation (e.g., W-9, purchase order, signed form). Deliverables: A fully functional PDF form template.
