Key Responsibilities: Team Social Worker: Work directly with patients and families (70% of the time). Patient and Family Support: Provide behavioral health support to high risk patients and their families, helping them navigate the complexities of kidney disease, and increase coping. Resource Coordination: Connect patients and families with community resources and support services, including financial assistance, behavioral health services, and care needs services. Advance Care Planning: Assist patients and families with goals of care conversations, and provide education on the advance care planning process. Education and Advocacy: Educate patients, families, and caregivers about available palliative and hospice care services, advocating for patient needs and preferences. Team Collaboration: Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients. Liaise with dialysis clinic staff and outside providers (30% of the time). Qualifications: Education: Master's degree in Social Work from an accredited institution. Licensure: State licensure as a Licensed Clinical Social Worker (LCSW) required. Experience: At least 2-4 years of clinical experience, preferably in healthcare settings. 2 years of renal experience preferred. Previous behavioral health experience and/or hospice or palliative care experience preferred. Bilingual preferred. Skills: Strong interpersonal and communication skills, ability to provide empathetic support in a remote work setting. Knowledge: Familiarity with end-of-life care principles, grief counseling techniques, behavioral health interventions, and community resources.
