Position Summary:
As a Clinical Team Lead, you will play a vital role in coordinating comprehensive patient care by assessing needs, developing personalized care plans, and facilitating services across healthcare providers. You will serve as a patient advocate, ensuring effective communication, resource utilization, and continuous monitoring to promote positive outcomes and enhance overall well-being. You will be accountable for supporting the day-to-day operations of the team, ensuring workflow efficiency and performance goals are met. This role requires fostering collaboration and teamwork among all department members.
Fundamental Components & Expectations:
- Serve as a content model expert for the regional team.
- Mentor team members on best practices, quality of interventions, problem resolution, and critical thinking.
- Provide real-time support to Clinical Management (CM) staff.
- Ensure adherence to plan sponsor performance targets.
- Articulate requirements to meet performance goals.
- Provide ongoing education to staff regarding opportunities and processes.
- Review and support adherence to policies and procedures.
- Provide support and education related to critical thinking and problem-solving.
- Escalate resolved cases and provide team education post-resolution.
- Communicate effectively with diverse audiences.
- Conduct routine data cleaning to ensure data accuracy and reliability in the EHR system.
- Collaborate with peers, leaders, and cross-functional partners to maintain quality metrics and identify process improvement opportunities.
- Organize and prioritize tasks effectively.
- Assist supervisors with reports to understand trends and maintain high levels of service.
- Design complex collaboration tools to facilitate effective clinical teamwork and information sharing.
- Identify opportunities to assist clinical case managers with scheduling, referrals, and secure document sharing.
- Install reporting tools and dashboards to identify trends, measure intervention impact, and inform quality improvement initiatives.
- Maintain an adjusted caseload.
- Assist with project logistics (agendas, meeting arrangements, minutes).
- Collect and write up success stories.
- Precept new staff as needed.
- Serve as a Subject Matter Expert regarding daily workflows.
- Promote a positive work environment that fosters teamwork.
- Consult frequently with the Regional Supervisor/Manager and maintain open communication.
Required Qualifications:
- Active, unrestricted KS State License in the applicable functional area (RN or Clinical BH license - e.g., LCPC, LCSW, LMFT, or LMHC).
- 3+ years of clinical experience required.
- Willing and able to travel up to 25% of time to meet members face-to-face within the western region of KS (Northwest, west-central, and Southwest).
- Reliable transportation required; mileage reimbursed per company policy.
Preferred Qualifications:
- 3+ years of managed care experience.
- Proficiency with computer skills, including navigating multiple systems and Excel experience.
- Time-efficient and highly organized.
- Exceptional communication skills.
- Ability to mentor and support staff to meet business needs.
- Working knowledge of problem-solving and decision-making skills.
- Working knowledge of medical terminology.
- Working knowledge of digital literacy skills.
- Ability to deal tactfully with customers and community.
- Ability to handle sensitive information ethically and responsibly.
- Ability to consider the relative costs and benefits of potential actions to choose the most appropriate option.
- Ability to function in clinical settings with diverse cultural dynamics of clinical staff and patients.
