Remote LPN Care Manager - Hybrid Role (Clay, Coosa, Randolph & Talladega Counties, AL)

We are seeking a compassionate and detail-oriented LPN Care Manager to join our team in a hybrid-remote role, requiring occasional on-site work in our office setting. As a Care Manager, you will play a vital role in ensuring seamless transitions across the health services spectrum for our patients, facilitating wellness and recovery through person-centered care plans.

In this position, you will work collaboratively with clinical and non-clinical staff, external providers, and community resources to assist clients in overcoming barriers to care, improving health outcomes, and promoting overall well-being. Your strong communication and problem-solving skills will enable you to effectively navigate complex patient needs and develop integrated care plans that address physical, behavioral, and social health requirements.

Your key responsibilities will include:

  • Assessing and re-evaluating patient needs to determine care requirements and preferences
  • Creating and implementing personalized care plans in conjunction with patients and caregivers
  • Identifying and addressing barriers to care, including insurance coverage issues and social determinants of health
  • Coordinating patient care activities, sharing information among treatment team members, and providing education on disease management and healthy living skills
  • Advocating on behalf of patients, tracking progress, and providing feedback to the treatment team
  • Maintaining accurate and timely documentation of care coordination in electronic health records

To succeed in this role, you should possess:

  • An Alabama Licensed Practical Nurse (LPN) license in good standing
  • At least three years of experience in nursing, with a preference for experience working with special populations
  • Proficiency in managing multiple priorities, accurate documentation, and electronic health records
  • Demonstrated ability to work collaboratively with multi-disciplinary teams and access community resources
  • Strong communication, problem-solving, and interpersonal skills
