Position: Community Health Worker... Location: Remote/WFH - Must be Resident of North Carolina Duration: 3 Month Contract to start, potential for extension and/or conversion to FTE Rate: $22.00/Hr Start Date: 11/04/2024 Important: Must be local resident of North Carolina Primary Responsibilities: As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate services are obtained via the Healthy Opportunities Pilot Program in addition to care coordination and member education. Primary Responsibilities:  Perform social needs assessment for members  Utilize the NCCARE360 platform for documentation and referral purposes  Clearly and effectively communicate with members and Human Service Organizations to arrange services  Linking members to applicable community resources  Telephonic outreach to targeted members  Proactively engage the member to manage their care  Provide member education  Support transitions of care  Create a positive and engaging experience for the member  Help to keep members compliant with their care plans  Partner with and support care team by completing delegated tasks  Knowledge and continued learning of community cultures and values  Conduct individual-level screenings to capture member's current health care needs (e.g. Transitions of Care, Health Risk assessments, Pilot Eligibility Service Assessment (PESA)  Engage member to participate in the screening process and collaboratively develop plan of care based on their individual needs, preferences, and objectives Required Qualifications:  High School Diploma / GED (or higher)  Must be a North Carolina resident  Must have a designated workspace inside the home with the ability to access high-speed internet  Must be proficient in Outlook and Microsoft Office  Must possess excellent organizational skills  Must be proficient in clear, customer service focused communication Would be a Plus to Have:  BachelorÂs Degree (or higher) in Social Work and / or Health Care Administration  Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical Assistant  CHW Accreditation  Experience working in Managed Care and/or Social Needs assessment  Knowledge of Medicaid population Hours: Remote Work/telecommute and must live in North Carolina (EST) Hours: 8 AM  5 PM EST, Monday-Friday, with a 1 hour lunch Equity Staffing Group is one of the fastest growing Native American owned staffing companies in the United States.
