Job Overview We invite you to become a pivotal member of our esteemed, NCQA-accredited behavioral healthcare organization as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC). This position offers the convenience of 100% remote work while empowering you to coordinate comprehensive patient care and navigate their healthcare journeys effectively. As a Clinical Care Coordinator (CCC), your focus will be on implementing Utilization and Population Health Management Programs and contributing significantly to our Quality Management initiatives. Key Responsibilities • Adhere to organizational policies and standards, including Apollo Medical Necessity Criteria, ASAM criteria, and URAC, NCQA, JCAHO, and HEDIS standards. • Collaborate closely with Health Plans or Medical Groups to maintain compliance with regulatory and clinical guidelines. • Utilize established clinical criteria to facilitate decision-making and uphold quality care standards. • Oversee and coordinate patient benefits and treatment plans to optimize outcomes within the Case Management/Chronic Care Management (CM/CCM) frameworks, as well as post-discharge care. • Actively engage in team meetings, training, and educational initiatives, including weekly clinical discussions. • Identify individuals eligible for Population Health Management (PHM) or Virtual Wards programs, assist in their enrollment, and conduct comprehensive initial assessments. Required Skills • Proficient computer skills and typing capability; familiarity with Microsoft Excel and Access is advantageous. • Comprehensive knowledge of URAC, NCQA, JCAHO, and HEDIS standards. Qualifications • Education: Master's degree in Psychology, Social Work, Counseling, or a related mental health field from an accredited institution. • Experience: A minimum of 3 years of direct patient behavioral healthcare experience and at least 2 years of case management, discharge planning, or utilization review experience. • Licensure: A current, unrestricted clinical license to practice in any U.S. state or jurisdiction; Certified AODA Counselor (CADC) certification is a plus.
