In this position, you will provide skilled nursing care in a home care environment. Your responsibilities will include assessing, planning, and documenting the homecare nursing process, as well as educating and providing instruction to patients and their families. You will also have supervisory responsibilities as assigned by the home health administrator and may be involved in special project management. As a part-time OB Home Care Nurse, you will positively represent the company to both internal and external customers while maintaining a professional and courteous manner. You will uphold the Optum core values in your work and adhere to company privacy guidelines, ensuring compliance with local, state, and federal regulations. Your services will be provided according to Optum policy and the prescriber's plan of treatment, and you will individualize patient assessments based on the care/services being provided and patient needs. You will be responsible for ensuring that interventions respect and encourage the patient's ability to make choices, and you will educate patients and families with a systematic approach to promote understanding of treatment and services. You will initiate and modify the plan of treatment/care pathway based on the analysis of information obtained, and coordinate care with patient services centers, pharmacists, and other healthcare professionals to promote continuity of care. Accurate communication of patient status information to the prescribing physician in a timely manner is essential, as is completing clinical record documentation per Optum policy in a timely and accurate manner. You may also provide service-related clinical support to key physicians and hospitals as directed, and participate in holiday and weekend duties as scheduled.

Requirements
Current unrestricted nursing license (RN) for state of residence.

1+ years of obstetrical nursing experience.

Proven ability to provide skilled nursing care for expectant mothers in the home care environment.

Proven ability to modify the plan of care based on reassessment and changes in prescriber orders.
,
Proven ability to continually assess the mothers' condition to ensure safety in the home environment.
,
Proven ability to establish trust and build long-term relationships with expectant mothers.
,
Proven ability to provide clinical support to key physicians/hospitals upon request.
,
Proven ability to utilize Windows programs including Word, Email, and Internet access.
,
Access to reliable transportation to travel to client and/or patient sites.
