Pair Team is on a mission to improve the wellbeing of underserved communities by connecting them to high-quality care.
Pair Team cares for the highest-need Medicaid recipients through a community-led model. We build local partnerships with shelters, food pantries, and other community-based organizations to turn them into a site of care. As a support system for the community, we provide wraparound clinical services, up-skill CBO staff to become Community Health Workers, and utilize our proprietary data-driven technology platform, Arc, for care coordination.
Through Medicaid MCOs, we provide healthcare for hard-to-reach, high-need individuals, while sharing healthcare dollars with community groups to expand their social support programs.

Pair Team is looking for a highly motivated full-time Referral Coordinator who will serve as a crucial liaison between our patients, providers, Plans, and internal team. You will manage and facilitate the referral process, ensuring that our patients receive prompt communication and timely care.
- Process, track, and coordinate patient referrals to ensure timely scheduling of appointments.
- Schedule PCP and/or specialist appointments for our patients.
- Coordinate with pharmacies (meds has been received, meds have been picked up, etc).
- Verify patient PCP and insurance (including prior auth requirements).
- Coordinate Release of Information.
- Assist with non-urgent patient follow up and/or information collection.
- 1+ years of backend healthcare and/or operations experience in a fast-paced healthcare environment.
- Confidence to own your role and propose solutions to be successful.
- A results driven attitude with a proven track record of exceeding performance indicators.
- Strong technical skills and comfort with utilizing multiple technology platforms simultaneously.
- Excellent proficiency with technology, like GoogleSuite applications, Slack, and Looker (or other comparable data tools).
- Requires a quiet, HIPAA compliant and internet connected space.
