THE OPPORTUNITY

ColigoMed is seeking a Medical Assistant (MA) to join our based Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) team in the US. The candidate must already be based in the Tampa Bay locality, Florida.

The Medical assistant (MA) reports to care coordinator nursing staff and is responsible for enrollment and onboarding of new patients and also monitoring a designated list of patients to support their adherence to approved patient care plans. The role reviews patient progress against care plan goals and guidelines and importantly helps them receive education, tips, and better disease management information. If issues are identified then these are escalated to a care coordinator.

Our team provides personalized care for each patient, with a dedicated team member who stays in touch as needed. This consistent contact helps build strong relationships and trust between patients and their care partners. These strong relationships between patients and the care team lead to better patient engagement, which in turn improves health outcomes.

Candidates must be willing to work hours that align with the Tampa Bay Clinic requirements usually Monday to Friday from 8.00am to 5.00pm ET. This is mandatory to meet our job requirements.

The successful candidate must already be located in the Tampa Bay locality and be willing to visit designated clinics as required. On average the person will work approx. 50% of the week working from home and 50% of the week working at a designed Tampa Bay Clinic.

RESPONSIBILITIES

● Obtain consent from patients for enrollment into ColigoMed monitoring programs (RPM/CCM)

● Educate patients on ColigoMed programs and help download and setup of the patient application

● Explain about monitoring device setup and if the patient has a device already guide them through taking the first device reading

● Refer any enrollment issues to the Care team for follow-up

● Follow enrollment processes and established guidelines

● Build effective relationships and rapport with designated patients as an extension of the medical practice and is an advocate for patient health

● Activate newly enrolled patients in the ColigoMed portal and support onboarding. Consult with patients upon receipt of devices to assist them with their use and obtain initial readings

● Complete a schedule of patient monitoring calls to review patient adherence to care plans and progress to assist patients achieve improved health outcomes

● Keep the care coordinators informed of all issues pertinent to the care plan as needed and any follow-up as directed

● Coordinate continuity of patient care with other healthcare facilities and care providers

● Document all communications (verbal or written) accurately on the Coligomed platform

● Adhere to documentation protocols and best practices for daily work logs, coding information for billing, escalation of patient issues and internal communications using the Coligomed platform

● Meet assigned patient targets to positively impact patient health outcomes

● Adhere to all relevant security and privacy regulations, such as HIPAA, and maintain strict confidentiality of patient information

● Contribute as a positive and supportive member of the Coligomed team

● Develop and maintain a strong knowledge of the remote care model and Coligomed platform and new developments in remote RPM and CCM solutions

● As required, travel from home to visit clinics in the Tampa Bay Area as required. In such cases an approved mileage rate for use of own car will be covered.

JOB REQUIREMENTS:

Education/Experience

● Active certification as a Medical Assistant from a nationally accredited institution (NHA, AAMA, NCCT, NAHP, etc)

● 1+ years of relevant healthcare experience in supporting patients remotely using a digital health platform including monitoring patients for a RPM and/or CCM program is required

Knowledge and Skills

● High level comprehension and communication skills with the ability to receive and convey information in a clear and understandable manner

● Strong commitment to provide uninterrupted services to patients in a remote working environment

● Strong organizational skills including prioritizing, scheduling, time management and meeting deadlines

● Demonstrated ability to interact with patients empathetically, earn trust and show sensitivity to their needs and concerns

● Familiarity with technology, computer proficiency, and the ability to quickly learn new software applications

● Ability to maintain a high level of professionalism, integrity, and ethical conduct when dealing with patients and their sensitive information

● Ability to work collaboratively in a fast-paced environment, demonstrating flexibility, adaptability, and a positive attitude

● Fluency in spoken & written English is mandatory with proficiency in Spanish language a plus

Note:

● A background check will be required

● A dedicated work area must be available at home that is separated from other living areas and enables information privacy to perform professional, quiet, and confidential work.

● Access to a high-speed internet service at home is mandatory

● As patient location evolves over time, flexibility in shift working hours may be required.

● Drivers license for travel to Tampa Bay area sites is required.

● A laptop and monitor will be provided.

Compensation

● An hourly rate ranging from $17.00 to $20.00 per hour. The actual position in hourly rate range will take into account factors including level of experience, education and skills. A mileage allowance will be paid for any approved travel to and from clinics.

Note: this is a 1099 role paid on hourly rate with no benefits. Over an initial three (3) to six (6) months period ColigoMed will assess and consider transition to full-time employment role that offers a benefits package including Medical, Dental and Vision Insurance, Paid Time off, Stock Options with career advance opportunities.
