- The pay for this position is $40.00 per hour.
 - Benefits available to full-time employees after 90 days of employment.
 - A 401(k) with company match available to full-time employees after 1 year of service.
 
- MondayFriday, 8:00 AM5:00 PM EST
 - Field role, with 80% travel required within a 2.53-hour radius from home for in-person member visits.
 - Mileage and lodging reimbursement provided.
 - Must be based in or near Rocky Mount, NC
 
- Manage a caseload of members with long-term care needs.
 - Conduct assessments with members, caregivers, and providers to determine support systems and care needs.
 - Monitor service delivery and follow up via in-person visits and phone contact.
 - Authorize and coordinate service referrals.
 - Identify deficiencies in care plans and ensure seamless service delivery.
 - Collaborate with discharge planners, physicians, and community partners.
 - Help members file and resolve complaints and appeals.
 - Document visits and maintain records using electronic medical health records.
 
- RN or LCSW license required.
 - Bachelors degree required.
 - 2+ years of experience in care management (46+ years preferred)
 - Experience in physical health, utilization management, long-term care, or home health
 - Proficiency in Microsoft Office and EMR software
 - Must have a valid drivers license.
 - Attendance is mandatory for the first 90 days.
 
- Background in home health, discharge planning, or long-term care
 - Strong documentation and communication skills
 - Ability to work independently while managing a mobile caseload.
 
Expected salary: $40 per hour
