As we expand, we are looking for a dedicated Occupational Therapist with experience in home health to join our dynamic team. • Assess, plan, and implement individualized treatment plans for patients in their homes.
• Provide therapeutic interventions to help patients improve or regain daily living skills.
• Collaborate with physicians, case managers, and other healthcare professionals to ensure coordinated, holistic patient care.
• Educate patients and families on how to enhance independence and improve their quality of life through therapy techniques.
• Document patient progress, treatment plans, and updates in accordance with regulatory standards.
• Monitor patient outcomes and adjust treatment plans as necessary to achieve the best results.
• Ensure that all therapy practices comply with state and federal regulations.

Qualifications:

• Valid and unrestricted Occupational Therapist (OT) license.
• Minimum of 2 years of experience as an Occupational Therapist, preferably in home health care.
• Strong knowledge of home health therapy practices and patient care.
• Excellent communication and interpersonal skills.
• Ability to work independently while maintaining a collaborative approach with the healthcare team.
• Detail-oriented with excellent time management and organizational skills.
• Commitment to providing high-quality, compassionate care.
• Experience with electronic health records (EHR) is a plus.
