The Licensed Practical Nurse is responsible for utilizing the nursing process to provide for optimum quality and continuity of nursing care to patients in the comfort of their residence.

    Responsibilities
  • Promptly complete and scan all mandatory documentation when a guest arrives, changes location, or leaves the premises.
  • Chart the nurses' documentation in a manner that effectively communicates the care provided to the patient and the impact of these interventions on the individual's well-being.
  • Address any discrepancies identified, including irregularities in physician orders, modifications in dietary needs, inaccuracies in charting, or other concerns, by reporting them to the Director of Nursing.
  • Ensure that all relevant data regarding unauthorized discharges is accurately entered in the guest's medical record following the established procedures.
    Qualifications
  • It is required to possess a valid LPN/LVN license that is unrestricted, or a Compact State license in states where applicable.
  • Prospective applicants must have accrued a minimum of 2 years of LPN/LVN experience.
  • Possessing the capability to educate patients and their families about multiple disease processes coupled with a thorough understanding of medications and experience in wound care.
