About the Position
We Care Home Health is seeking a highly skilled and dedicated LPN or RN to provide specialized care for an extended care patient in a home setting. This role involves working night shifts, ensuring the patient's safety, comfort, and medical stability. Key Responsibilities
- Delivering specialized care for a patient with trach, vent, and G-tube needs, including monitoring the patient's respiratory and nutritional health during your shifts
- Administering medications and tube feedings as prescribed by the patient's healthcare provider
- Performing routine assessments, interventions, and adjustments as necessary to ensure the patient's safety and comfort
- Communicating with the care team and maintaining accurate patient records to ensure continuity of care
- Developing strong, supportive relationships with the patient and their family to provide emotional support and education

Essential Qualifications
To be successful in this role, you will need to possess the following essential qualifications:
- Current Colorado LPN or RN license
- Experience with trach, vent, and G-tube care preferred, but not required
- Current BLS/CPR certification
- Strong clinical and communication skills, with the ability to work independently in a home care setting
- A compassionate, patient-focused approach to nursing, with a commitment to delivering high-quality care
