About the Role

Key Responsibilities
- Conduct comprehensive Biopsychosocial Assessments to understand patients' needs and develop personalized care plans.

 - Engage in discharge and transition planning to facilitate smooth care transitions.
 - Foster client and family engagement in all aspects of social work intervention, including advanced healthcare planning, palliative care, and hospice or end-of-life care.
 - Identify and address elder/vulnerable adult abuse, trauma, neglect, and exploitation, ensuring the safety and well-being of our patients.
 - Facilitate benefits and resource acquisition, assisting clients and families in understanding related policies, eligibility requirements, and financial and legal issues.
 
Qualifications and Skills

Essential Qualifications
- A Licensed Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling, or a Licensed Ph.D., or an RN with 2 years of experience in behavioral health.
 - Active, independent, and unrestricted licenses in the state of Missouri.
 - 2 years of experience in a related mental health setting.
 - Proficiency in Microsoft Word, Excel, and Outlook.
 - A dedicated work area that ensures information privacy and the ability to keep company-sensitive documents secure.
 - Willingness to travel up to 75% within the specified territory and work Monday through Friday, with occasional overtime as needed.
 - A valid driver's license and reliable transportation.
 - Residence within 90 minutes from Springfield, MO, with access to a UnitedHealth Group-approved high-speed internet connection.
 
Preferred Qualifications
- 2 years of experience in long-term care, home health, hospice, public health, or assisted living.
 - 1 year of clinical or case management experience.
 - Familiarity with electronic charting and arranging community resources.
 - Background in managing populations with complex medical or behavioral needs.
 - Field-based work experience, particularly in home settings.
