Position Overview
The Cross‑Market Care Coordinator (CMCC)—also known as the Skilled Inpatient Care Coordinator (SICC)—plays a pivotal role in guiding patients through the complex landscape of post‑acute care. Operating remotely, you will conduct functional assessments, collaborate with multidisciplinary teams, and orchestrate discharge plans that align with each patient’s goals and regulatory standards.
Key Responsibilities
Clinical Assessment & Coordination
- Serve as the primary conduit between patients, families, and health‑care personnel, ensuring timely and smooth transitions of care.
- Perform comprehensive Skilled Nursing Facility (SNF) assessments using clinical expertise and CMS criteria at admission and throughout the patient’s stay.
- Review Length of Stay (LOS) targets, outcome metrics, and discharge objectives with providers, patients, and caregivers.
- Complete all concurrent SNF reviews, updating authorizations promptly to reflect changing clinical status.
- Participate in weekly SNF rounds, delivering accurate, up‑to‑date information to senior managers and medical directors.
Collaboration & Communication
- Engage with interdisciplinary teams—including physicians, referral coordinators, discharge planners, social workers, physical therapists, occupational therapists, and speech‑language pathologists—to develop optimal discharge strategies.
- Facilitate weekly telephonic outreach to patients, families, and caregivers, providing education, support, and clarification on care plans.
- Attend patient/family care conferences and document key decisions and action items.
- Coordinate peer‑to‑peer reviews with naviHealth Medical Directors, ensuring alignment with utilization management protocols.
Utilization Management & Compliance
- When delegated, evaluate referral requests for continued SNF stay, forwarding complex cases to licensed physicians for NOMNC issuance.
- Maintain up‑to‑date knowledge of federal and state regulations, including CMS guidelines, HIPAA, and health‑plan policies.
- Ensure strict confidentiality of protected health information (PHI) in all documentation and communications.
- Document all clinical activities accurately in the nH coordinate system, adhering to audit‑ready standards.
Operational Excellence
- Manage a defined caseload efficiently, employing strong time‑management and prioritization skills.
- Conduct daily census reviews to identify barriers, develop mitigation strategies, and support colleagues as needed.
- Analyze monthly dashboards, readmission reports, and quarterly metrics with Clinical Team Managers to drive continuous improvement.
- Support new contract start‑ups by sharing expertise and ensuring smooth knowledge transfer among staff.
- Participate in cross‑training initiatives, expanding proficiency across multiple contracts and client sites.
Essential Qualifications
- Active, unrestricted clinical license in Arkansas: Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech‑Language Pathologist (SLP).
- Minimum 5 years of direct clinical experience in acute, post‑acute, or home‑health settings.
- Demonstrated ability to conduct functional assessments and interpret CMS criteria.
- Proven proficiency in telephonic communication and virtual care coordination.
- Strong ethical foundation with a commitment to patient confidentiality and regulatory compliance.
