They are seeking a Patient Financial Advocate to assist self-pay patients by assessing their financial needs, determining eligibility for assistance, and facilitating the application process for available resources. Responsibilities • Assesses clients’ financial needs of self-pay patients. • Interviews customers to determine eligibility for assistance through state, county or local organization. • Keeps knowledge of referral sources current. • Assesses financial situation and discusses financial alternatives with clients and families. • Evaluates feasibility (i.e., financial, willingness) of services and resources. • Assists patients/families in seeking resources in order to obtain needed care. • Discusses alternatives with patient and family. • Assists customers filling out government program applications. • Assists customers in securing available funding within the community to help them settle their outstanding obligation for the service received. • Documents education, referrals and the resolution of problem. • Follows-up to ensure success of plan. • Assists social workers in planning for patients’ financial needs after discharge. • Meets one-on-one with patients and families to individualize discharge plan and after-care. • Communicates recommended follow-up treatment to interdisciplinary treatment team. • Uses knowledge of referral agencies to facilitate continued treatment or placement. Education Requirements • Bachelors Degree: Bachelor’s degree in Social Work or related field preferred. • Bachelor’s degree in Social Work or related field preferred.
