About the Role
We're seeking a compassionate and experienced RN Health Coordinator to join our team in Oahu, HI. As a field-based coordinator, you'll play a critical role in assessing, planning, and implementing care strategies for patients in the local community. You'll work closely with an elite and dynamic team to ensure that patients receive the right services to meet their needs. Key Responsibilities
- Assess, plan, and implement care strategies: Develop individualized care plans that are directed toward the most appropriate, least restrictive level of care for each patient.
- Identify and initiate referrals: Connect patients with social service programs, including financial, psychosocial, community, and state supportive services.
- Manage care plans: Serve as a single point of contact for patients and manage their care plans throughout the continuum of care.
- Communicate with stakeholders: Share health-related information with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
- Advocate for patients: Represent patients' needs and choices to ensure they receive the best possible care and support.
Requirements and Qualifications
Essential Qualifications
- Current, unrestricted RN license: A current, unrestricted RN license in the state of Hawaii is required.
- 2+ years of clinical experience: At least 2 years of clinical experience is required, with experience working directly or collaborating with services for long-term care, home health, hospice, public health, or assisted living.
- Intermediate level of proficiency with MS Office: Intermediate level of proficiency with MS Word, Excel, and Outlook is required.
- Ability to travel: Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.
- Current access to internet: Current access or ability to obtain internet access via a landline is required.
Preferred Qualifications
- Bachelor's degree or higher: A Bachelor's degree or higher is preferred.
- CCM (Certified Case Manager): CCM certification is preferred.
- Experience with electronic charting: Experience with electronic charting is preferred.
- Experience with arranging community resources: Experience with arranging community resources is preferred.
- Field-based work experience: Field-based work experience is preferred.
- Case Management experience: Case Management experience is preferred.
- Experience working within the community health setting: Experience working within the community health setting is preferred.
- Background/experience with at least one of the following: Behavioral/mental health, psychiatric nursing, housing/homeless outreach, community resource linkages, pregnancy, pediatrics, or disease management.
- Background in managing populations with complex medical or behavioral needs: Background in managing populations with complex medical or behavioral needs is preferred.
- Background in mental health or experience working with serious mental illness: Background in mental health or experience working with serious mental illness is preferred.
- Bilingual in Cantonese or Mandarin: Bilingual in Cantonese or Mandarin is preferred.
- Psych background including community nursing: Psych background including community nursing such as PHN or IP psych facility is preferred.

- Competitive salary: A competitive salary range of $28.03 to $54.95 per hour for Hawaii residents.
