Job Overview

We are seeking a highly skilled and compassionate Home Hospital Physician to provide hospital-level acute care to patients in the comfort of their own homes. This innovative role offers the opportunity to practice medicine in a unique and dynamic setting, working collaboratively with an interdisciplinary team of healthcare professionals. As a Home Hospital Physician, you will be responsible for providing medical decision-making, patient evaluations, documentation, and oversight of the Home Hospital team. You will also have the opportunity to participate in care redesign initiatives and mentored research and innovation projects.

Key Responsibilities

  • Provide home visits within a defined radius around site hospitals according to the Home Hospital care model
  • Provide direct care (in-person or virtual), counseling, and teaching to a designated patient population in the inpatient and home settings
  • Available to patients for urgent issues overnight, typically via virtual collaboration with a community paramedic
  • Perform complete histories and physical examinations
  • Oversee and/or collaborate with multidisciplinary team members according to organizational policies and practices
  • Collaborate with nurse practitioners and physician assistants on mutual patient care
  • Order, interpret, and evaluate appropriate laboratory and diagnostic tests
  • Develop appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings
  • Order medications and write prescriptions according to organizational and regulatory policies and procedures
  • Consistently provide high-quality and timely documentation, including admission and progress notes, procedure notes, and discharge summaries

Non-Clinical Responsibilities

  • Demonstrate professional collaborative and consultative relationships with other clinicians
  • Participate in departmental and organizational committees and programs as appropriate
  • Attend all required meetings, in-services, and professional training
  • Maintain superior interpersonal and communication skills as a member of the healthcare team
  • Remain actively involved in continuing education with a commitment to self and departmental growth
  • Adhere to all established safety policies, procedures, and precautions

Qualifications and Requirements

To be successful in this role, you will need to possess the following qualifications and skills:

  • Internal Medicine BC/BE
  • Proficient in inpatient medicine, virtual care, and multidisciplinary team-based care
  • Willing to make home visits (alone or with other team members) within a defined radius around site hospitals
  • Open-minded, creative, team-oriented, expert communicator
  • Ability to work in a high-stress, dynamic, busy clinical setting with changing organizational climate
  • Ability to be self-directed and to demonstrate initiative in addressing the responsibilities of the position
  • Ability to utilize appropriate problem-solving and conflict resolution skills
  • Energetic and quality-driven with a proven ability to be a productive member of a team dedicated to providing optimal patient care
  • Interest in working as part of a service that is committed to innovation, rapid cycle testing, and process improvement
  • Active Advanced Cardiopulmonary Life Support (ACLS) certification
  • Maintaining credentialing at Mass General Brigham hospitals to support patient care needs in a system-wide Home Hospital program

Preferred Qualifications

  • Competency in (and/or willingness to learn) bedside procedures appropriate to the patient population
