CVS Health is seeking a compassionate and experienced Field Case Manager RN to join our team in Cumberland County and surrounding areas. As a Field Case Manager, you will play a vital role in delivering high-quality healthcare services to our members, working closely with them to develop and implement care plans that meet their unique needs. Job Summary

This is a full-time remote opportunity with 50-75% travel in Cumberland and surrounding counties. The schedule is Monday - Friday, 8am-5pm EST. As a Field Case Manager RN, you will be responsible for:

  • Conducting face-to-face visits in members' homes, utilizing comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Program (D-SNP/FIDE)
  • Developing, implementing, supporting, and promoting health service strategies, tactics, policies, and programs that drive the delivery of quality healthcare to our members
  • Coordinating and collaborating care with members, primary care physicians, and other care team participants
  • Scheduling and attending interdisciplinary meetings and advocating on members' behalf to ensure proper and safe discharge with appropriate services in place
  • Developing care plans and authorizing services in a cost-effective manner within the MLTSS/FIDE benefit
  • Documenting accurately and timely in members' electronic health records
  • Mentoring new hires once a level of proficiency has been attained in their role

Key Responsibilities

Member-Centric Care

  • Conduct comprehensive assessments of members' medical, behavioral, and psychosocial needs
  • Develop and implement individualized care plans that address members' unique needs and goals
  • Collaborate with members, families, and care teams to ensure seamless care coordination

Care Planning and Coordination

  • Develop and implement care plans that meet members' unique needs and goals
  • Coordinate with members, primary care physicians, and other care team participants to ensure comprehensive care
  • Advocate on members' behalf to ensure proper and safe discharge with appropriate services in place

Clinical Judgment and Critical Thinking

  • Utilize advanced clinical judgment and critical thinking skills to facilitate appropriate physical, behavioral health, and psychosocial wrap-around services
  • Analyze complex medical and social data to inform care planning and decision-making

Requirements

Education and Licensure

  • Active and unrestricted RN license in New Jersey
  • Minimum of an Associate's degree in Nursing required

Experience and Skills

  • 3+ years of clinical practice experience in a hospital setting, alternative care setting, or home health
  • Must reside in Cumberland County or an adjacent county
  • Willing and able to travel 50-75% in Cumberland County and surrounding counties
  • Successful completion of the NJ Choice certification is a contingency of employment

Preferred Qualifications

  • Case management and/or discharge planning experience
  • Managed Care experience
  • Crisis intervention skills
