Job Summary
As a Medical Social Worker, Home Health Per Diem, you will play a vital role in our interdisciplinary care team, working closely with patients, families, and healthcare professionals to provide comprehensive care and support. Your expertise will focus on assessing and addressing the social and emotional needs of our patients, developing personalized plans of care, and connecting them with community resources to promote recovery and well-being. Key Responsibilities
- Conduct comprehensive assessments of patients' social and emotional needs, developing personalized plans of care to address these needs and promote recovery and well-being.
- Provide direction and supervision to Social Worker Assistants as required, ensuring that all medical social services are delivered in compliance with company, government, and professional standards.
- Collaborate with the interdisciplinary care team to develop and implement patient-centered plans of care, participating in care coordination activities and discharge planning processes.
- Assess patients' relationships, financial resources, and available community resources, providing guidance and support to access these resources and address any barriers to recovery.
- Instruct patients and families on coping with social and emotional responses to illness or injury, providing ongoing assessment and support to address changing needs and responses to treatment.
- Maintain accurate and timely documentation of patient interactions, progress, and outcomes, submitting reports and conferencing with other healthcare disciplines as required.
Essential Qualifications
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Current social worker licensure in the state of practice, as required by state law or regulation.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
- Valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Excellent oral and written communication and interpersonal skills, with the ability to read, write, and speak fluent English.

- Knowledge of medications and their correct administration.
- Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Ability to communicate effectively with patients, families, and healthcare professionals at all levels of the organization.
- Current knowledge of community resources, including housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility.
