As a Field Behavioral Health Care Coordinator/SPMI, you'll have the opportunity to make a meaningful difference in the lives of our members, working closely with a talented team of Behavioral Health Specialists, RNs, and Social Workers to ensure they receive the highest quality care.About the Role
As a Field Behavioral Health Care Coordinator/SPMI, you'll be responsible for assessing, planning, and implementing individualized care strategies for patients, identifying and initiating referrals for social service programs, and managing the care plan throughout the continuum of care as a single point of contact. You'll also communicate health-related information to all stakeholders, advocate for patients and families, and visit Medicaid members in their homes and other settings.Key Responsibilities:
- Assess, plan, and implement individualized care strategies for patients.
- Identify and initiate referrals for social service programs, including financial and psychosocial support.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate health-related information to all stakeholders to ensure quality coordinated care.
- Advocate for patients and families to ensure their needs and choices are represented.
- Visit Medicaid members in their homes and other settings, including community centers and hospitals.
Requirements
To be successful in this role, you'll need to possess one of the following: RN in the state of Texas, Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Masters Social Worker (LMSW). You'll also need to have 2+ years of experience working within the behavioral health setting, intermediate proficiency in MS Word, Excel, and Microsoft Outlook, and reliable transportation and the ability to travel up to 75% of the time in the assigned region.Preferred Qualifications:
- Medicaid Waiver experience.
- Case Management and/or Care Coordination experience.
- Behavioral health experience.
- Field-based work experience.
- Experience with electronic charting.
- Background in managing populations with complex medical or behavioral needs.
- Knowledge of community resources.
